29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: (a) 123 Marion Street, Hamburg, Pennsylvania - An employee suffered a work-related injury on January 28, 2022 as a result of slipping on ice and falling to the ground. The injury required hospitalization, and the employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this in patient hospitalization to OSHA on January 31, 2022.) No abatement certification or documentation required
