29 CFR 1910.219(c)(3): Vertical or inclined shafting seven (7) feet or less from floor or working platform was not enclosed with stationary casing(s) in accordance with paragraphs (m) and (o) of this section: On or about February 16, 2022, employees working in the Slurry room were exposed to entanglement hazards because: a. the vertical drive shaft used to turn the Mixer for Dissolver #3 was not guarded or enclosed in accordance with the standard. b. the vertical drive shaft used to turn the Mixer for Dissolver #7 was not guarded or enclosed in accordance with the standard.
