29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Milling Area - On or about February 10, 2022, an employee suffered an amputation, while performing a tool change on a Sharp Milling Machine, and a lockout device was not affixed to an energy isolating device to prevent an unexpected energization.
