29 CFR 1926.502(d)(8):Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two: a) Worksite, 211 North Main Street, Stillwater, New York, Roof of Facility: On or about February 09, 2022, an employee was attached to a SALA SecureaSpan Horizonal Lifeline System while installing metal decking along the roof of a commercial building approximately 36-feet above the ground. The SALA SecureaSpan Horizonal Lifeline System was not installed as the manufacturer's instructions and used under the supervision of a qualified person.
