29 CFR 1910.28(b)(3)(ii) Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems; On or about March 3, 2022, employees were exposed to tripping hazards from open floor holes in the shake-out/green sand floor area: (a) Floor Hole #1, ~8 inches by 6 3/4 inches, and; (b) Floor Hole #2 (with pipe), ~18 1/4 inches by 7 inches, and; (c) Floor Hole #3, ~8 inches by 9 inches.
