29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage were made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked: a. On or about 01/20/2022, at the warehouse - employees were exposed to struck-by and caught-in hazards because Powered Industrial Truck (PIT) operators did not have a clear view of the warehouse aisles because their path view was obstructed by merchandize pallets that were stacked at different heights and have been placed over the designated walk and verifiers area of the main aisle. b. On or about 01/20/2022, at the warehouse - employees were exposed to struck-by and caught-in hazards because pallets were obstructing the pedestrian and verifiers walk and standing area of the main aisle.
