29 CFR 1910.212(a)(2) Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible: a) On or about January 13, 2022, the employer failed to provide effective guarding to protect an employee working at or near the JIT small body lathe asset #10252. An employee's glove became entangled in the machine's rotating parts, which subsequently caused an amputation to the left-hand index finger, when the employee attempted to set up and center a metal tube on the rotating parts of the small body lathe.
