29 CFR 1910.178(k)(1):The brakes of highway trucks were not set and wheel chocks placed under the rear wheels to prevent the trucks from rolling while they are boarded with powered industrial trucks. Location: 6801 West Side Ave. North Bergen, NJ 07047 - Shipping Warehouse Dock 70 a) Employee exposed to fall and crushing hazards; employee was operating a forklift inside a cargo trailer when the truck driver began to drive away, exposing the forklift operator to roll backwards and fall out approximately 4 feet to the ground surface. The incident occurred on or about 12/27/21.
