29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) PR-10 machine; On or about January 12, 2022 and at times prior to the employer exposed employees to amputation hazards in that employees were allowed to conduct maintenance on the PR machines without first utilizing the LOTO procedure.
