29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Instance 1 location: storeroom entryway; On or about 1/14/2022, employees were not protected from a fire hazard when a Comfort Zone portable space heater was plugged into an extension cord. The manufacturer's labeling states that the cord needs to be plugged directly into a wall receptacle and never use with an extension cord. Instance 2 location: retail area near main entryway On or about 1/14/22, employees were exposed to an electrical shock hazard when using a loose electrical outlet box which was designed to be mounted on a secure structure.
