29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about December 23, 2021, employees were exposed to fall hazards of 24 feet 6 inches to a lower level while walking to and accessing the permanent ladder in the area of the Skip Hoist Construction Project located at 8826 US-24 Mapleton, IL. 61547, and the employer did not ensure an adequate fall protection system was used.
