29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: (a) On January 7, 2022, while in the process of installing precast chimney caps on a residential property located at 1842 S Ogden St. Denver, CO, two employees working from a tubular steel framed scaffold that was not fully planked were not protected from a fall hazard. Both employees fell approximately 28 feet from the top of the work platform to the ground below, when the wooden planks failed.
