29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about December 16, 2021, the employer did not report to OSHA a work-related hospitalization prior to the elapse of the 24 hour reporting period after learning about the hospitalization. The incident was not reported to OSHA until December 21, 2021.
