Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. Location - 10 Lyman Street, Waltham, MA. On or about December 20, 2021, employees were exposed to a fall of at least fourteen feet above lower level from residential roof where fall protection was not used.
