29 CFR 1910.106(b)(6):In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition. Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat: (a) On or about December 18, 2021 at or near 8953 63rd Street NW in Ross, North Dakota, for the employees exposed to fire and explosion hazards resulting in the fatality of one employee and the hospitalization of another, while engaged in rim wheel servicing activities with the use of a torch where a tanker containing residual vapors from methanol water was venting in the facility.
