29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 7, 2021, at the facility in the leg cell department, employees performing servicing and/or maintenance activities such as, but not limited to, unjamming a mini end-former where the safety interlock that interrupted the electrical power to the machine when the cover was opened had been disabled. No written lockout procedures were developed, documented or utilized. The mini end-former has multiple energy sources and an effective alternative means of protecting employees was not provided.
