29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 7, 2021 the employer failed to ensure that procedures for the isolation of hazardous energies of the #7 extrusion line cuttoff machine were utilized prior to service and maintenance taking place. Employees were not removed from and able to be within the hazardous area of the machine while positioning and feeding material through the cutting apparatus. Hazardous energies present on the equipment mechanical, electrical, and pneumatic.
