1910.147(d)(2) The machine or equipment was not turned off or shut down using procedures established for the machine or equipment. An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. (a) Mill Production Area - On or about December 2, 2021, one of the cant pickup cylinders was sticking, resulting in one side lifting and resting at an angle. Prior to attempting to perform maintenance work on the equipment, the potential energy was not removed from the equipment, resulting in the cant pickup dropping down and striking an employees arm.
