Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush and amputation hazards while operating a Cincinnati (BP-05) Ton Hydraulic Press Brake with the Presence-sensing Device (light curtain) turned off or made ineffective leaving the point of operation without guarding. a) Special metals department; On or about December 3, 2021 and times thereto, the employer exposed employees to machine guarding hazards in that employees were able to bypass the setting on the control panel which rendered the light curtain ineffective causing an amputation.
