29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) On or about 11/23/2021 at a utility project located along Genesee Road in East Concord, New York; employees hydro-excavating for new utility poles had placed their truck along the shoulder and partially in the east bound lane of the road. Employees had not been trained in work zone set up including, but not limited to, early warning, transition areas, redirection of traffic from normal path when encroaching on the lane of travel, and devices and methods to control traffic.
