OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to stuck-by and crushing hazards from unsecured concrete forms: a. 90 Farley Road Millburn, NJ 07041 On or about 11/23/2021, employees were exposed to struck-by and crushing hazards while storing multiple concrete forms on an A -frame rack. Employees worked in the fall shadow of unsecured concrete forms that were stored in A-frame racks on uneven ground. Each unsecured form weighed approximately 88 pounds, and employees working in the fall shadow of the forms were exposed to struck-by and crushed-by hazards.
