29 CFR 1926.651(b)(4): While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees: (a)H & W Contracting, L.L.C. at 271st Street and Kerslake Place, Tea, SD 57064: On or about and at times prior to November 16, 2021, the employer did not ensure that multiple utilities, including a gas main, water line, electrical line and communication line were supported or protected. This condition exposed employees to potential hazards associated the release of hazardous energy from damaged utility lines.
