29 CFR 1910.333(c)(3)(i)(A)(1): When an unqualified person is working in an elevated position near overhead lines with voltages to ground rated at 50kV or below, the location was not such that the person and the longest conductive object he or she could contact came closer than 10 ft. (305 cm) to any unguarded, energized overhead line: (a) Safeway Inc., 4600 E. Stapleton Dr. South, Denver, CO 80216: On or before November 12, 2021, the employer did not ensure that each employee working in an elevated position near overhead lines with voltages to ground rated at 50kV or below was protected from injury in that an employee working from an aerial lift contacted an energized electrical line rated at 6.2 kV which resulted in severe burns to the employee. This condition exposed employees to electric shock and thermal burns.
