29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. At the Anchor Glass Container Corporation facility located at 2121 Huron Street, on the #3 Tank Cullet Return Conveyor System: On or about November 13, 2021, the employer exposed three employees to amputation hazards, in that the #3 Tank Cullet Return conveyor system was not de-energized and locked out at the drive motor, as employees were servicing the conveyor's tail-end pully system.
