29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Birmingham lathe LA-2; On or about November 10, 2021 and at times prior thereto, the employer exposed employees operating the lathe to amputation and caught-in hazards in that the employer permitted employees to have their hands inside the danger zone of the equipment while manually sanding a rotating shaft.
