29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On or about November 1, 2021, the employer failed to report an amputation to the USDOL-OSHA within the 24-hour reporting period. Texas Roadhouse, Inc. was previously cited for a repeat violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1526744, citation number 1, item number 1 and was affirmed as a final order on June 2, 2021, with respect to a workplace located at 1885 Waddle Road, State College, PA 16803. Texas Roadhouse, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1292233, citation number 1, item number 2 and was affirmed as a final order on February 25, 2019, with respect to a workplace located at 5010 North Desert Blvd, El Paso, TX 79912.
