29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about November 5, 2021 at approximately 8:30 PM, at the Kroger Distribution Center located on 701 Gellhorn in Houston, TX, an employee was operating a Crown model 5545 reach truck with an added chain on the lifting forks to retrieve scrap metal out of a dumpster, without prior written approval from the manufacturer.
