29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. Warehouse(s): On or about 10/22/2021, and times prior, the employer exposed employees to struck by hazards, in that employees were operating the Raymond 8410 End Rider Pallet Jack power industrial truck and the employer did not ensure employees completed training on the Raymond 8410 End Rider Pallet Jack prior to operating power industrial truck-pallet jack. Variety Wholesaler, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910. 178(l)(4)(ii(B)), which was contained in OSHA inspection number 1281797, citation number 1, item number 2 and was affirmed as a final order on 5/23/2018, with respect to a workplace located at 60 Herring Rd Newnan, GA 30265
