Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. Located at 115 Enterprise Parkway, West Jefferson, Ohio: On and/or before November 1, 2021 the employer failed to ensure Lockout/Tagout procedures were utilized for the TC 0112 conveyor. The employer developed a written energy control procedure. However, supervisors did not enforce the energy control procedure as maintenance personnel routinely removed the belly pans guards of the conveyors during normal production operations. This practice exposed employees to moving machine parts and in-going nip point hazards created by the moving rollers, which resulted in the hospitalization of an employee as a result of an employee's arm being caught between the snubber roller and the belt of the conveyor. Torrid LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1455348, citation number 1, item number 1 and was affirmed as a final order on March 20, 2020, with respect to a workplace located at 115 Enterprise Pkwy, West Jefferson, OH 43162.
