29 CFR 1910.134(a)(2):A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee:(a) (LOCATION) (OPERATION) (EXPOSURE TO WHICH MATERIALS) (SPECIFIC LOCATIONS) On or about February 8, 2022, the employer did not require respiratory protection for employees working at the presses. The employer did not require respiratory protection, and did not implement elements of respiratory protection program including, but not limited to: a written program, medical evaluations, fit testing, and training. a.) On or about February 8, 2022, an employee working primarily on Press 600T-3, was exposed to 0.49mg/m3 of copper fume over a sampling period of 455 minutes, approximately 4.9 times the limit of 0.1mg/m3. Exposure calculations included a zero increment for the time not sampled. b.) On or about February 8, 2022, an employee working primarily on Press 80T-1, was exposed to 0.50mg/m3 of copper fume over a sampling period of 428 minutes, approximately 5 times the limit of 0.1mg/m3. Exposure calculations included a zero increment for the time not sampled.
