29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: On or about October 25, 2021 Location: 131 Mamaroneck Ave. White Plains, NY 10601 eastside of building 3. a) Employee fell into 14 inch underground pipe that was covered with Filler fabric and 3/4 stone. Employee was hospitalized for leg injury. b) Employees were working in area where 14 inch underground pipe that was covered with Filler fabric and 3/4 stone exposing employees to fall hazard.
