OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts: Instance a) At the facility's Bay 1, on or about October 12, 2021, a two post surface mounted Challenger lift, SN 170900000169129, was operated without the locking mechanism working. Instance b) At the facility's Bay 1, on or about October 27, 2021, a two post surface mounted Challenger lift, SN 170900000169129, was operated without being inspected and certified since July 2020. Instance c) At the Bay 2, on or about October 27, 2021, a two post surface mounted Rotary lift, SN DGP17B0019, was operated without being inspected and certified since July 2020. Instance d) At the facility's Bay 3, on or about October 27, 2021, a two post surface mounted Challenger lift, SN170200000157599, was operated without being inspected and certified since July 2020. Instance e) At the facility's Bay 4, on or about October 27, 2021, a two post surface mounted Rotary lift, SNDGP15H0001, was operated without being inspected and certified since July 2020. Instance f) At the facility's Bay 5, on or about October 27, 2021, two post surface mounted Rotary lift, SN DHU15H0001, was operated without being inspected and certified since July 2020. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: Ensure all automotive lifts are operated and frequently inspected and make repairs in accordance with the ANSI/ALI ALOIM 2008, standard for automotive lifts, section 4.4.2. In addition, ascertain the inspection is performed timely by a qualified lift inspector, with signature and post the inspection certificate on the lift.
