29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) AAA ENVIRONMENTAL, INC. at 4777 Marketplace Drive, Johnstown, CO 80534: On or before October 10, 1021, the employer did not ensure that employees were protected from injury in that employees installing insulation on the ceiling inside of a commercial building were elevated above the ground from the basket of a JLG aerial lift while not wearing fall protection. This hazard exposed the employees to a fall hazard of approximately 20 feet.
