Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. On or about October 7, 2021 and times prior there to, the employer did not develop or implement written lockout/tagout procedures for each type of equipment or machine located within the manufacturing facility. Employees performing maintenance work on the equipment such as, but not limited to, the Box Making Machine, Cardboard Bailers, Powered Industrial Trucks and other job scope related machines were exposed to electrical shock and amputation hazards caused by the unexpected release of stored energy.
