(a) At the facility, back room/mezzanine area: On or prior to 10/6/21, a Werner 10 foot step ladder used to access the mezzanine area did not extend at least 3' above the landing surface, exposing employees to a fall hazard of approximately 10' while climbing to or from the mezzanine.
