29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) Groundworks Colorado, LLC DBA Foundation Repair of Western Colorado at 638 Meadow Rd in Snowmass Village, CO 81615: On or about October 1, 2021, and at times prior, employees working in trenches up to 10 feet 6 inches deep had not received training related to trenching hazards or how to recognize and control those hazards.
