29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed. A) International Timber & Veneer, in the shop, on the planner machine on or about September 20, 2021 : An employee was using an A-frame step ladder during maintenance work on the machine. The employee used the step-ladder to transition their foot onto the machine to straddle the work area between the ladder and the machine. The employee fell from the ladder and suffered an amputation injury. NOTE: In Accordance with ANSI A14.5 - Safety Requirements for Portable Reinforced Plastic Ladders: Section 9.3.6 Side loading: Ladders are not designed for excessive side loading. The ladder should be kept close to the work. Marking #4A � Lean into or hold onto ladder to maintain 3 points of contact while working. � Keep ladder close to work. Move ladder as needed. Do not walk or move ladder while standing on it. � Do not climb on or off ladder from the side unless ladder is secured against tipping.
