29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Low Volume Cell, Banner projection welder model number 2AP150A12, serial number 7052 - On or about September 23, 2021, the banner projection welder did not have adequate guarding at the point of operation where the two-hand controls and the electronic light curtain were not functioning properly, resulting in an employee suffering a fingertip amputation with their hand in the point of operation. NHI Mechanical Motion DBA New Hampshire Industries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1509705, citation number 1, item number 1 and was affirmed as a final order on April 26, 2021, with respect to a workplace located at 35 Connecticut Riverbend Place, Claremont, NH 03743.
