29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. At a workplace located at 1207 S. Scenic Ave., Springfield, MO: Employees were exposed to amputation hazards from the Lico Trim Saw. There were no machine specific lockout tagout procedures for shutting down, isolating, blocking and securing the Lico Trim Saw when there is electrical , pneumatic and stored energy. The saw blade keeps rotating after being locked out. On or about September 23, 2021, an employee received two partial finger amputations when there were no machine specific lockout tagout procedures when unjamming wood and conducting maintenance on the Lico Trim Saw.
