29 CFR 1910.333(c)(3):When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: (a) Lubeck PSD - On or about September 22, 2021, an employee was operating a truck crane within 10 feet of an 18KV overhead line which was not deenergized, grounded, or insulated, exposing employees to electrocution and shock hazards.
