The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective. a) On or about September 7, 2021 at Tri-State Storage in Chesapeake Ohio, the employer did not ensure that employees working on equipment such as but not limited to the Viking VK-2 115 volt, rolling gate controller, did not become part of the path to ground.
