29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) K.N.S. Services, Inc. located at 600 N Pickaway St, Circleville Ohio 43113: On and preceding August 26, 2021, the employer did not ensure that a work-related incident resulting in a hospitalization was reported within 24 hours. At approximately 1:45 pm on 8/26/2021, an on-duty field technician was injured on the job and hospitalized. The employer did not report the hospitalization to the OSHA office until 9/16/2021 at 12:23 pm.
