29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300: A) Frozen Specialties Inc., 720 W Barre Road, Archbold, Ohio 43502: On or about September 16, 2021, the employer failed to ensure that the OSHA Form 300 for calendar year 2021 contained a complete description in column F. The 2021 300 log did not define the object that directly caused the injury to the employee.
