Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that they were exposed to amputation hazards: a) Ellwood National Forge; Tool Room - On or about 9/3/21 and at times prior thereto, an employee was exposed to caught in and amputation hazards while performing sanding and polishing procedures on a rotating shaft by holding emery cloth on the spinning surface by hand. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
