29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Location - GBG 50 ton swager machines: On or about September 9, 2021, employees were exposed to crushing and amputation hazards while operating Swager presses which were not adequately guarded, resulting in one employee suffering the amputation of a fingertip.
