29 CFR 1910.140(c)(13)(ii):Anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two: a) 1200 Block of Lodge Lane in the rear of 502 River Road, Wilmington, DE 19809: On or about September 8, 2021, the qualified employee did not ensure the arborist installed and utilized a fall protection system that maintain a safety factor of two. The employee fell approximately 15 feet to ground and suffered serious injuries.
