29 CFR 1910.178(l)(4)(ii)(B):Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: Employees were exposed to struck-by hazards. An employee working in the welding shop was seriously injured when struck-by a fork truck operated by an employee who has had approximately 20 near miss incidents and accidents documented by the employer. The fork truck operator was not provided retraining on fork truck operations.
