OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation hazards while shaking the Hollymatic Super Patty machine to get it to start. Location: Prep Room/Cutting Room An employee using a Hollymatic Super Patty Machine reached under an inadequate guard in order to shake the machine to get it to start, the machine turned on and two of the employee's fingertips were amputated when caught between the top adapter plate and the lower mold plate.
