29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part: (a) Jobsite located at 3515 Midvale Avenue Philadelphia, PA, 19129: The employer did not maintain an accident prevention program to help protect their employees from hazardous working conditions. Employees were exposed to soil collapse hazards while removing stones out of an excavation with approximately 11.5 feet deep that was improperly benched. Condition was observed on or about August 27, 2021.
