29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: A. Aulfab, LLC, 1018 Matzinger Rd., Toledo, Ohio 43612: On or about August 13, 2021, the employer did not ensure employees were adequately protected from entering the point of operation of the CNC Hydraulic Press Brake, Model UHSB155-13, S/N W1902-256H. Employees operating the machine in this condition was exposed to crushing hazards when cycling the machine in or near the point of operation.
